Form 4137

Use Form 4137 to calculate the social security and Medicare tax owed on tips you did not report to your employer, including any allocated tips shown on your Form(s) W-2 that you must report as income.

  1. Go to Federal Taxes > Review
  2. Click I’d like to see the forms I’ve filled out or search for a form.
  3. Type Form 4137 in the search box.
  4. Click Add form next to Form 4137/8919 – Social Security and Medicare Taxes.
  5. Enter in the applicable information and click Save.

Form 8919

Use Form 8919 to figure and report your share of the uncollected social security and Medicare taxes due on your compensation if you were an employee but were treated as an independent contractor by your employer.

  3. Type Form 8919 in the search box.
  4. Click Add form next to Form 4137/8919 – Social Security and Medicare Taxes.
  5. Enter in the applicable information and click Save.
