Overview

Form 8915-D is used to report distributions received by individuals adversely affected by a qualified 2019 disaster, as listed in Table 1 at the end of the form instructions. The distribution must meet the criteria outlined under the Qualified 2019 Disaster Distribution Requirements, also provided in the instructions.

Add Form 8915-D

To add the Form 8915-D

  1. Go to Federal Taxes.
  2. Click Review.
  3. Click I’d like to see the forms I’ve filled out or search for a form.
  4. Enter Form 8915-D in the search box and click on Form 8915-D – Qualified 2019 Disaster Retirement Plan Distributions to add and open the form.
  5. Enter your information and click Save.
