Use Form 4506 to:

• Request a copy of your tax return, or
• Designate a third party to receive the tax return.

Add Form 4506

To add the Form 4506 – Request for Copy of Tax Return:

1. Go to Federal Taxes.
2. Click Review.
3. Click I’d like to see the forms I’ve filled out or search for a form.
4. Enter 4506 in the search box and click on Form 4506 – Request for Copy of Tax Return to add and open the form.
5. Enter your information and click Save.
