LA – Disaster Loss Claim

Use the LA Disaster Loss Claim screen to submit a Louisiana disaster loss claim when personal property has been damaged or destroyed in a qualifying event.

Key Information to enter:

  • Mailing Address: Address, ZIP, City, State
  • Location of Property Destroyed: Address, ZIP, City, State, Parish
  • Address change: Checkbox, if applicable
  • Resident Since: Date the taxpayer became a Louisiana resident
  • Disaster Details: Name and date of the disaster
  • Preparer Information: Name and phone number (if applicable)
  • Cause of Destruction: Select from Flood, Wind, or Other (with a description field)
  • Type of Home: House, Apartment/Condo, Mobile Home
  • Areas of the Home That Received Damage:
    • Kitchen
    • Breakfast area
    • Patio
    • Living room
    • Den
    • Outdoor structure
    • Dining room
    • Garage/Carport
    • Bedrooms (number of rooms)
    • Bathrooms (number of rooms)
    • Other (with description)

All fields on this screen are override-enabled (green fields) to allow for precise entry.

How to Add LA Disaster Loss Claim

  1. Go to the State taxes section.
  2. Select I’d like to see the forms I’ve filled out or search for a form.
  3. In the drop list, select Louisiana.
  4. Select the General tab.
  5. Select Disaster Loss Claim to add and open the form.
  6. Complete the screen.
  7. Click Save to apply changes.
