Overview 

Rhode Island law requires most residents to maintain qualifying health coverage throughout the year. If you were uninsured for all or part of the year—or are claiming an exemption—you must complete the RI – Individual Health Care Mandate Form as part of your state

Who Needs to Complete This Form? 

You should complete the Individual Health Care Mandate Form if: 

  • You are requesting an exemption from the coverage requirement. 
  • You need to calculate a shared responsibility payment for months you were uninsured and not exempt. 

How to Report 

  1. Navigate to the State Taxes section on the left-hand side of the screen. 
  1. Click the link: “I’d like to see the forms I’ve filled out or search for a form.” 
  1. In the dropdown menu, select Rhode Island
  1. Click on the General tab. 
  1. Click on “Individual Health Care Mandate Form” to open the form. 
  1. Enter all applicable information.  
  1. Click ‘Save‘ once complete.
