Overview
New Jersey’s Easy Enrollment Health Insurance Program offers individuals and families a quick way to connect with affordable health coverage options when filing their Form NJ-1040. By completing Schedule EZE, you allow the New Jersey Department of Banking and Insurance to contact you about health insurance options through the Get Covered NJ marketplace.
Who Should Complete Schedule EZE?
You should complete this form if:
- You are uninsured or want help evaluating your health coverage options.
- You want the state to use your tax return information to assess your eligibility for low-cost or no-cost health coverage, such as NJ FamilyCare or Get Covered NJ plans.
- You consent to being contacted by the state regarding potential health insurance enrollment.
Note: Completing Schedule EZE does not automatically enroll you in a health plan, but it begins the process of determining your eligibility for coverage.
How to Report
- Navigate to the State Taxes section on the left-hand side of the screen.
- Click the link: I’d like to see the forms I’ve filled out or search for a form.
- In the dropdown menu, select New Jersey.
- Click on the General tab.
- Select Schedule EZE – Easy Enrollment Health Insurance to open the screen.
- Enter all applicable information.
- Click Save once complete.

- Your information will be used only for the purpose of determining health coverage eligibility.
- This form is optional, but it may help you gain access to more affordable healthcare.
- If you are already enrolled in Medicaid (NJ FamilyCare) or through the Get Covered NJ marketplace, you do not need to complete this form.
