General Information
The GA – 5347 – Statement of Person Claiming Refund screen is used to request a refund on behalf of a deceased taxpayer. This form is required if the taxpayer passed away and was due a refund for the listed tax year.
You’ll use this screen to enter the claimant’s:
- Name and Social Security Number
- Address
- Relationship to the deceased
- Legal status (e.g., surviving spouse, executor, or other qualified heir)
You’ll also need to indicate whether the claimant meets one of the three categories:
- A. Surviving spouse requesting reissuance of a refund check
- B. Court-appointed or certified personal representative
- C. Other qualified person claiming on behalf of the estate
A final checkbox confirms that the claimant is an heir and meets the qualifications stated in the affidavit.
How to Add Form 5347
- Click on the State Taxes section on the left-hand side of the screen.
- Click the link “I’d like to see the forms I’ve filled out or search for a form.”
- Use the drop list to select Georgia.
- Under the Other tab, select 5347 – Statement of Person Claiming Refund.
- Complete the screen.
- Click Save.
