If you owe interest or penalties for underpayment, late payment, or late filing of your Massachusetts state taxes, you may need to complete the Interest and Penalty Worksheet on your return. This form helps calculate the amount due and ensures compliance with Massachusetts Department of Revenue (DOR) regulations.
Generally, you should complete the Interest and Penalty Worksheet if:
- You are filing your return after the due date and have a balance.
- You are subject to failure-to-pay, failure-to-file, or underpayment penalties.

Add MA Interest and Penalty Worksheet
To add the Interest and Penalty Worksheet:
- Go to State Taxes.
- Click I’d like to see the forms I’ve filled out or search for a form.
- Select Massachusetts.
- Click on the Other tab.
- Click on Interest and Penalty Worksheet to add and open the form.
- Enter your information and click Save.
