Wisconsin – Identity Protection (IP) PIN

Overview 

If you have been issued an Identity Protection Personal Identification Number (IP PIN) by the IRS or Wisconsin Department of Revenue; you must include it on your Wisconsin state return. The WI – IP PIN form allows you to securely provide this information during your filing process. 

What is an IP PIN? 

An IP PIN is a six-digit number issued to eligible taxpayers to help prevent fraudulent use of their Social Security Number (SSN) on tax returns. 

  • The IP PIN is used to verify your identity when filing electronically. 
  • Without the correct IP PIN, your return may be rejected or delayed. 
  • Both the Taxpayer and (if filing jointly) may need to provide their individual IP PINs. 

Who Should Include This Form? 

You should complete the WI – IP PIN form if: 

  • You received an IRS CP01A Notice containing your annual IP PIN. 
  • You were issued a Wisconsin Department of Revenue IP PIN for security. 
  • You are filing jointly, and either you or your spouse has been assigned an IP PIN. 

How to Report 

  1. Navigate to the State Taxes section on the left-hand side of the screen. 
  1. Click the link: I’d like to see the forms I’ve filled out or search for a form.
  1. In the dropdown menu, select Wisconsin
  1. Click on the General tab. 
  1. Click on IP Pin to open the form. 
  1. Enter all applicable information.  
  1. Click Save once complete.
