Overview
If you are using direct debit to pay your North Carolina income taxes, it recommended to provide your contact information in case the North Carolina Department of Revenue (NCDOR) needs to reach you regarding your payment. This ensures prompt resolution if issues arise (such as incorrect bank info, payment failure, or scheduling concerns).
To do so, use the Direct Debit Contact Information screen as part of your NC D-400 Individual Income Tax Return.
When to Provide Contact Information
You should complete this form if:
- You are authorizing a direct debit (electronic funds withdrawal) from your bank account for:
- Balance due payment,
- An estimated tax payment, or
- An extension payment, and
- You want to ensure NCDOR can reach you directly in case there are issues processing the payment.
Note: This information is used only for payment-related issues and does not affect how NCDOR contacts you for other tax matters.
How to Report
- Navigate to the State Taxes section on the left-hand side of the screen.
- Click the link: I’d like to see the forms I’ve filled out or search for a form.
- In the dropdown menu, select North Carolina.
- Click on the General tab.
- Select Direct Debit Contact Information to open the screen.
- Enter all applicable information.
- Click Save once complete.
