GA – 5347 – Statement of Person Claiming Refund

General Information 

The GA – 5347 – Statement of Person Claiming screen is used to request a refund on behalf of a . This form is required if the taxpayer passed away and was due a refund for the listed tax year. 

How to Add 

  1. Click on the State Taxes section on the left-hand side of the screen. 
  2. Click the link I’d like to see the forms I’ve filled out or search for a form
  3. Click on the dropdown menu for Georgia
  4. Select the Other tab. 
  5. Click on 5347 – Statement of Person Claiming Refund to open the screen. 
  6. Complete the required claimant information and selections. 
  7. Click Save
